Following is a chronological listing of disciplinary
actions taken by the Board from February 28,
2003, through February 20, 2004. Each entry
includes the name, certificate or registration number
(if any), and last known significant address of
the respondent; the disciplinary sanction imposed;
a brief description of the basis of the disciplinary
sanction; the effective date of the disciplinary
sanction; and whether the disciplinary sanction was
taken pursuant to an Order (O), Adjudication
and Order (A&O), or Consent Agreement and Order(C&O).
